AwardsIf you have shown exceptional talent and dedication to delivering outstanding healthcare or customer service at any level within community pharmacy, or you wish to nominate a colleague who has, here’s your chance to nominate for or enter the prestigious Pharmacy in Focus Awards. In conjunction with the Ulster Chemists’ Association (UCA), these professionally judged awards are highly valuable accolades and will be presented at the President’s Ball on 17 January 2009.

HOW TO ENTER

Simply complete the expression of interest entry form opposite and return it to us by 7 November 2008.

Application forms will be sent to all entrants whose award will be judged on the basis of their written submission.
If you have entered the Customer Service award categories you will not receive an application form. Instead, our
mystery shoppers will visit your business.

WHO CAN ENTER?

• The Awards are open to pharmacy contractors, managers or staff working in Northern Ireland community
pharmacy.
• You can enter yourself, your staff, your business or you can nominate a business or individual(s).
• Entrants may enter as many categories as they wish, providing categories entered are relevant.

YOU CAN WIN

• A specially commissioned trophy.
• Two complimentary places at the Ulster Chemists’ Association President’s Ball and Award Ceremony.
• An extensive media and public relations campaign for yourself and/or your business.
• £1000 prize money per category (excluding Outstanding Contribution Award which will receive a special presentation gift).

Competition Rules:

Ulster Chemists’ Association Executive Committee members cannot enter their business nor be nominated for these awards, however heir staff are eligible to enter or be nominated. Entrants may enter as many categories as they like providing they are relevant. Winners will be announced at UCA President’s Ball, 17 January 2009 at Ramada Hotel, Belfast.

All entries must be accompanied by a completed entry form, which should be signed by the person responsible for submitting the entry. All entries will be acknowledged on receipt.

The judges’ decision is final and no correspondence shall be entered into. All entrants must be available to attend the Awards Ceremony on 17 January 2009 and be willing to be photographed for promotional purposes.
